Climbing stairs after a heart attack, stent or bypass
Stairs are hard work for the heart, so take them slowly at first. Go one step at a time, and rest when you need to. It is fine to be a little out of breath. You should still be able to talk.
The talk test and the warning signs: the British Heart Foundation (BHF).
Why stairs feel hard
Climbing lifts your whole body, step after step. That is hard work for the heart. Doctors often use two flights of stairs as a guide. The NHS says sex puts about the same strain on the heart as climbing two flights of stairs.
How to take the stairs
- Go slowly. One step at a time.
- Hold the rail.
- Rest on a step, or at the top, when you need to.
- Do not carry heavy bags up the stairs in the early weeks.
Pace yourself through the day. Break up busy tasks with rest.
After a stent or heart attack
Start with gentle walking, even for a few minutes. Walk every day until it feels easier. Then walk a little longer. Stairs get easier as you get fitter. For about a week after a stent, do not lift heavy things.
After bypass surgery
Nurses and physios help you get up and move from the day after the operation. At home, start going for walks as soon as you feel well enough. Most people get back to most normal activities after about 6 weeks. Do not do heavy lifting for 3 months.
Breathless on the stairs: when to worry
Being more out of breath than you would expect can be a sign of a heart problem, such as heart failure. Tell your doctor if:
- you get more breathless on the stairs than before,
- you get breathless with everyday tasks, or when you lie down,
- you feel chest tightness, dizziness or a racing heart when you climb.
If the chest pain is bad and does not go away with rest, call 108.
What happens at Cardiac Rehab

- A doctor checks you first.
- You get an exercise plan made for you.
- You exercise for 90 minutes, 3 times a week, for 6 months, under physician supervision.
- Step by step, the exercise gets harder as you get stronger.
Why it helps with this
In rehab, you learn your safe level with the team watching your heart trace. So stairs at home stop being a guess.
Dr Muralidar’s advice: supervised exercise can start as early as 1 week after a stent. After bypass, it can start after about 20 days at home.
A 2026 review of 107 studies looked at people after a heart attack, a stent or bypass. Those who did cardiac rehab had 28% fewer heart attacks. They had 35% fewer hospital stays.
Heart guidelines in the US (AHA and ACC) and in Europe (ESC) give cardiac rehab their highest grade: Class I, Level A.
Cardiac Rehab, Jubilee Hills, Hyderabad runs these sessions.
