None of those people felt ill. Physician supervised, exercise based cardiac rehab acts on the things behind that change. Across 63 trials it lowered deaths from heart disease from 10.4 per cent to 7.6 per cent.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The damage nobody can feel<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Nearly 480,000 adults. One blood marker, GlycA, which rises when the body has been inflamed for a long time. Then the heart scans.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In the people with the most inflammation, the heart had already changed. Walls a little thicker. Chambers a little smaller. Filling a little less well.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Take the highest fifth for inflammation. They carried a 43 per cent higher risk of heart attack and stroke than the lowest fifth. It names what can be changed: smoking, and weight.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">What it does not say is who helps a person change them. Nor what to do once the heart has already begun to stiffen.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That is the whole of my work.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">How cardiac rehab acts on this exact problem<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Physician supervised, exercise based cardiac rehab is not advice to exercise. It is a prescription, written for one heart, watched while it is taken. Here is what it does to the problem above.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">It raises the work the heart can do<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Effort is graded upward week by week, under watch, so the muscle adapts instead of being overloaded. A heart asked to fill and empty at a sustainable pace, most days, fills better than one that is not.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">It works on the blood vessels<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Regular aerobic training improves how arteries relax and open. Those vessel linings are the first tissue that long term inflammation damages, which is why this matters here rather than in general.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">It goes straight at the two things the study names<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Weight falls with sustained aerobic work, and it stays down because the effort is repeatable. Stopping smoking is part of the programme, not a leaflet handed over at discharge.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">And it is safe for the people who need it most<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">A person whose heart is already stiff is usually told to rest. Under supervision, with the heart watched while the effort rises, that same person can train. That is the difference between a gym and a rehab programme.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The numbers<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A Cochrane review of exercise based cardiac rehab in coronary heart disease, 63 trials and 14,486 people:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Deaths from heart disease fell from <strong>10.4 per cent to 7.6 per cent<\/strong>. Hospital admissions fell from <strong>30.7 per cent to 26.1 per cent<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The part that should trouble any doctor<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Cardiac rehab is one of the least used treatments in heart medicine. Most people who qualify for it are never sent. It does not come in a box and nobody markets it, so it is quietly left out of the plan.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">There is a long, silent window between the first change in a heart and the first symptom. That window is what this programme is built for.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Questions patients ask<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">What is cardiac rehab, exactly?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">A prescription for exercise, written for one heart and watched while it is taken. Effort is raised step by step under a doctor&#8217;s eye, alongside help with weight, smoking, blood pressure and sugar. It is not advice to go to a gym.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Is it only for people who have had a heart attack?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">No. It strengthens hearts after a heart attack, after angioplasty, after bypass and in heart failure. It is also used before heart surgery, and as a way to prevent a first heart attack in somebody at high risk.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">My heart is already weak. Is exercise safe for me?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">That is exactly who supervision is for.
